Breast Augmentation with Fat Transfer: Everything You Need to Know About Breast Fat Grafting

aumento de pecho con grasa

Breast fat transfer has become a well-established natural alternative to breast augmentation with implants. This technique allows breast volume to be increased using fat from your own body, typically harvested from areas such as the abdomen or thighs. In this article, we explain how the procedure works, what results you can expect, and which patients are particularly suitable candidates for this treatment.

What Is Breast Augmentation with Fat Transfer?

Breast augmentation with fat transfer, also known as breast fat grafting, involves removing fat from an area of the body where there is a localized fat deposit—such as the abdomen, flanks, or inner thighs—and transferring it to the breasts to increase their volume and improve their shape.

This technique offers a double benefit:

  1. Enhances body contours in the donor area through liposuction
  2. Increases breast volume naturally, without the need for implants.

How Is the Procedure Performed?

Fat Harvesting

The first step is to perform liposuction on the donor area. It is generally recommended to use “untouched” areas—those that have not undergone previous surgery—as the fat from these regions tends to be more viable and has a higher likelihood of surviving the transfer.

The most common donor areas are:

  • Abdomen
  • Flanks
  • Inner thighs

These areas typically provide higher-quality fat cells with greater cellular viability, making them particularly suitable for fat transfer procedures.

Fat Purification

Once harvested, the fat is processed and purified. There are two main methods used:

  1. PureGraft® filtration system: separates the liquid components from the cellular portion of the fat.
  2. Coleman technique: uses centrifugation to separate the liquid, oily, and adipose components.

The purified fat cells, isolated from the other components, are then prepared for injection into the breasts.

Fat Injection into the Breasts

The fat is carefully and evenly injected into the breasts to enhance their shape, cleavage, upper pole fullness, and projection. The goal is to achieve a natural, harmonious result that complements the patient’s body proportions.

How Many Cup Sizes Can Be Increased?

The increase in breast volume depends on two main factors:

  1. The amount of fat available for transfer.
  2. The number of fat transfer sessions performed.

With a single fat transfer procedure, it is usually possible to achieve an increase of one to two bra cup sizes. If more volume is desired, additional procedures can be performed, provided there is sufficient donor fat available. However, it is uncommon to undergo more than three fat transfer procedures.

The final result also depends on the initial breast size and the quality of the skin, particularly its ability to expand and accommodate the transferred fat.

Is the Fat Reabsorbed?

Yes. Approximately 40% of the injected fat is reabsorbed during the first six to eight weeks after surgery. This expected reabsorption is taken into account by the surgeon when planning the procedure and typically occurs evenly, without causing irregularities or uneven areas.

The fat that remains after the first month to month and a half is generally considered permanent. For this reason, the volume seen immediately after surgery will gradually decrease until the final result stabilizes.

Is the Result Long-Lasting?

Yes, the results are long-lasting. Once the initial reabsorption phase has passed, the remaining volume is generally maintained over time. However, it is important to remember that the fat transferred to the breasts is living tissue and can be affected by significant weight fluctuations.

In other words, if you experience substantial weight loss, you may notice a reduction in breast volume. Conversely, if you regain your usual weight, the volume in the breasts is likely to increase again as well.

Can a Breast Lift and Fat Transfer Be Performed at the Same Time?

Yes, it is common to combine a breast lift with fat transfer. However, injecting large volumes of fat during a breast lift is generally not recommended.

This is because a breast lift reshapes the breast and involves incisions within the tissue, which can temporarily reduce blood supply to the area. If a large amount of fat is injected at the same time, there is a greater risk that the transferred fat will not survive and will be reabsorbed.

For this reason, when fat transfer is combined with a breast lift, the fat is typically used to refine breast shape and enhance the overall result rather than to achieve a significant increase in cup size.

Hybrid Breast Augmentation: Combining Implants and Fat Transfer

For patients who do not have enough body fat for a full fat transfer procedure, or who prefer a more natural result without the need for large implants, hybrid breast augmentation may be an ideal option.

This technique combines:

  1. A small breast implant to provide the desired base volume.
  2. Fat transfer to enhance the cleavage, improve upper pole fullness, and soften the edges of the implant.

The result is a natural-looking breast shape with smoother contours and less visible implant edges.

For slim patients with limited fat reserves, minimally invasive breast augmentation techniques such as Mia Femtech™ Breast Harmonization and Preservé™ Breast Augmentation are also available. These innovative approaches offer additional options for achieving breast enhancement with minimal scarring and a faster recovery.

Replacing Breast Implants with Fat Transfer

This procedure is very common among patients who wish to remove their breast implants and restore their breasts using their own tissue for a more natural result.

How Is the Procedure Performed?

  1. The breast implants are removed.
  2. If necessary, the periprosthetic capsule—the scar tissue that naturally forms around the implant—is also removed.
  3. Liposuction is performed to harvest fat from a donor area.
  4. The harvested fat is purified and prepared for transfer.
  5. The purified fat is then injected into the breasts.

If the skin has stretched significantly or the nipples have descended over time, the procedure can be combined with a breast lift to improve breast position and shape.

The transferred fat is used to enhance breast contour, improve cleavage, restore fullness and roundness, and add projection—particularly in patients who have had larger implants removed.

Postoperative Care and Recovery

Recovery after breast augmentation with fat transfer is generally straightforward and not particularly painful. However, some discomfort is common, especially in the areas where the fat was harvested.

In the Breast Area

  • Bruising may appear during the first few days after surgery.
  • Patients are usually advised to wear a supportive bra that provides adequate support without excessive compression.

In the Donor Area (Liposuction Site)

  • Bruising is also common in the donor area.
  • A compression garment should be worn as recommended by the surgeon.
  • Manual lymphatic drainage massages are often advised to help speed up recovery, reduce swelling, minimize the risk of fibrosis and tissue irregularities, and improve the final body contour.

Who Is a Good Candidate for Breast Augmentation with Fat Transfer?

Patients Who No Longer Want Breast Implants

This procedure is often chosen by women who wish to remove their breast implants and restore volume using their own tissue.

A typical candidate may be a woman in her 40s or 50s who:

  • Has had children.
  • Feels that her implants no longer reflect how she sees herself.
  • Finds that breast implants appear too large after natural weight gain over the years.
  • Requires implant replacement due to rupture or simply wishes to have the implants removed.

Young Patients Who Prefer to Avoid Implants

Breast fat transfer can also be an excellent option for younger women who:

  • Prefer not to have breast implants.
  • Wish to enhance both their body contours and breast volume using only their own fat.
  • Are looking for a result that feels and looks natural.

Three Signs You May Be an Ideal Candidate

You may be a good candidate for breast augmentation with fat transfer if:

  1. You do not want breast implants.
  2. You are seeking a subtle, natural-looking enhancement rather than a significant increase in volume.
  3. You have areas of localized fat that could benefit from liposuction and serve as donor sites for the procedure.

RECOMMENDED TREATMENTS

The volume and shape of the breasts can be improved by implanting the patient’s own fat, without the need for a breast implant.

Breast lift

Breast lift surgery allows you to recover the position and firmness of the breasts that, due to reasons such as breastfeeding or weight loss, have caused them to sag.
