Each breast has its own anatomical characteristics and may also change throughout life as a result of factors such as genetics, hormonal changes, pregnancy, breastfeeding, ageing, or weight fluctuations.

Understanding the different types of breasts can help you better understand your own anatomy and, when there is a particular characteristic you would like to change, assess which options may be most suitable.

Small breasts, large breasts, tuberous breasts, breast asymmetry, or sagging breasts all require completely different approaches. That is why, in breast surgery, there is no single technique that can be applied to every patient: treatment must be adapted to:

the anatomy

the volume

the quality of the tissues

and the desired outcome.

<picture></picture>

Types of breasts according to shape

Breast shape depends on several factors:

how the breast tissue is distributed

the position of the nipple and areola

the shape of the chest

the position of the inframammary fold

and the amount and quality of the skin

Based on these characteristics, we can identify different types of breasts.

Round breasts

We refer to round breasts when the breast volume is distributed similarly across all four quadrants of the breast and the nipple is positioned approximately in the centre.

This distribution provides a uniform appearance in both the upper and lower parts of the breast.

Tuberous breasts

Tuberous breasts are a breast shape abnormality that can present with different degrees and characteristics. A tuberous breast has a constriction in the lower pole of the breast.

This constriction prevents the breast gland from expanding and properly filling the lower part of the breast, affecting its shape.

Depending on the characteristics of the breast, different surgical techniques can be used to correct tuberous breasts. In some cases, a breast augmentation may be performed while also reshaping the gland. In other cases, it may be necessary to remove part of the glandular tissue, perform a breast lift, or use the patient’s own fat to improve specific areas and achieve a more harmonious shape.

The technique will always depend on the specific characteristics of the breast.

Teardrop-shaped breasts

Teardrop-shaped breasts have a greater concentration of volume in the lower part of the breast.

As an aesthetic reference, an approximate distribution of 55% of the volume in the two lower quadrants and 45% in the two upper quadrants has been described.

This 55/45 proportion has been described in scientific literature as an aesthetic reference for breast proportions and continues to be used as one of the reference parameters in breast plastic surgery.

However, these proportions should be understood as an aesthetic reference rather than a rule applicable to every patient. The result should always be adapted to the individual’s anatomy and proportions.

Asymmetrical breasts

Breast asymmetry is common and can present in different ways.

There may be a difference in volume between the two breasts, a difference in the position of the nipple and areola, different amounts of skin, a difference in the position of the inframammary fold, or a combination of several of these factors.

Volume asymmetry

In some cases, one breast has a different volume from the other.

Correction should take into account not only how much volume is present on each side, but also the shape and characteristics of the tissues.

Asymmetry in nipple position

It is also possible for both breasts to have a similar volume while one nipple is positioned lower than the other.

This can occur, for example, after breastfeeding. If one breast has stretched more than the other, there may be a greater amount of skin on one side.

When the bra is removed, this difference in skin can cause one breast to sit slightly lower, creating asymmetry in nipple position.

Asymmetry of the inframammary fold

The inframammary fold, which approximately corresponds to the area where the underwire of a bra rests, may also differ in height.

It may be positioned higher on one breast and lower on the other.

When breast surgery is performed, these differences must be taken into account during surgical planning.

Asymmetry caused by the shape of the chest

In some patients, the asymmetry is not limited to the breasts but is also present in the structure of the chest itself.

Conditions such as scoliosis or pectus excavatum can cause one side of the chest to be more sunken than the other.

In these cases, correction may be more complex because there is an underlying structural anatomical asymmetry that cannot always be compensated for with an implant alone.

Wide-set breasts

Wide-set breasts can have different causes and therefore cannot always be corrected in the same way.

When the separation is mainly due to a lack of volume, it may be easier to visually reduce the distance between the breasts.

However, some patients have a divergent chest configuration. The curvature of the ribs causes the implants to rest on a surface that slopes outwards and, as a result, the breasts may appear more laterally positioned.

In certain cases, a hybrid breast augmentation may be considered, combining implants with the patient’s own fat to improve the cleavage area and create the visual appearance of more defined cleavage.

East-west breasts or outward-pointing nipples

In so-called east-west breasts, the nipples point more laterally.

Correction can be complex because nipple position is determined by the underlying anatomy.

When an implant is positioned centrally in relation to the chest, a nipple that is already lateralised may continue to point outwards and may even become more visually noticeable depending on the projection of the implant.

In certain cases, adjustments can be made to try to visually improve nipple position, such as skin correction or a periareolar mastopexy.

However, depending on the anatomy, the lateralisation may not be completely corrected. This type of breast can also occur in association with certain tuberous breasts with limited breast development.

Ptotic or sagging breasts

Breast ptosis refers to sagging of the breast and can occur to different degrees.

There may be a simultaneous descent of the nipple and gland, or pseudoptosis, in which the nipple remains in an appropriate position but the glandular tissue descends towards the lower part of the breast.

Depending on the position of the nipple and gland, ptosis can be classified into different degrees.

In certain mild cases, placing an implant can add volume and create a lifting effect. When sagging is more significant, it may be necessary to perform a mastopexy to reposition and reshape the breast.

Types of breasts according to volume

In addition to shape, breast volume is another key factor that determines the characteristics of the breasts.

Breasts can range from having limited glandular development to being very large and potentially causing physical discomfort.

Small breasts

Small or hypoplastic breasts have limited volume and a smaller amount of glandular tissue.

When the main concern is solely a lack of volume, it can be corrected through breast augmentation.

This augmentation can be performed:

with implants

with the patient’s own fat

or using a hybrid approach that combines both options.

When there are no other significant shape abnormalities, correction generally focuses on increasing volume while maintaining proportions that are appropriate for the patient’s body.

Medium-sized breasts

A breast with a medium volume may not require a significant increase in size, but the patient may wish to modify certain aspects of its shape, such as increasing upper-pole fullness.

In these cases, breast augmentation or lipofilling may be considered.

When there is sufficient glandular tissue and the patient is seeking a moderate increase in volume, techniques such as Mia® or Preservé™ may allow smaller implants to be used and covered by the existing breast tissue, helping to achieve a natural result.

Large breasts

When breast volume is significant, some patients may consider a breast reduction.

In hypertrophic breasts, the weight of the gland can place a significant load on the tissues and may also be associated with varying degrees of sagging.

In cases of gigantomastia, the volume can cause significant physical discomfort, including:

back pain

marks on the shoulders from bra straps

skin irritation in the inframammary fold.

Friction, perspiration, and prolonged contact between the breast and the skin of the abdomen can contribute to irritation, particularly during warmer months.

In these cases, breast reduction can reduce the volume and weight of the breasts, with the aim of improving both their proportions and the patient’s comfort in everyday life.

What factors influence breast shape and size?

Breast shape does not necessarily remain the same throughout life.

Genetics, physical development, hormonal changes, pregnancy, breastfeeding, menopause, and weight fluctuations can all alter the volume, shape, and position of the breasts.

Genetics and physical development

Genetics influence breast shape and volume, although their effects can vary.

There may be significant differences between women within the same family, as breast characteristics can be inherited from both the maternal and paternal sides.

Even so, there may be a degree of familial tendency in characteristics such as breast volume.

Puberty and breast development

Puberty is one of the main periods of breast change.

From the onset of the first menstrual period, the process of physical development and maturation continues, associated with the hormonal changes characteristic of this stage.

When certain breast abnormalities are present, such as tuberous breasts or significant asymmetry, they should be assessed individually, taking into account the degree of development reached.

Pregnancy and breastfeeding

Pregnancy and breastfeeding can considerably alter breast volume and shape.

During these stages, the breasts undergo changes associated with both hormones and variations in volume.

The extent of these changes may also depend on weight fluctuations during pregnancy.

For this reason, it is important to follow the gynaecologist’s recommendations regarding weight gain and to take proper care of the skin during pregnancy and the postpartum period.

<div style="text-align: center; margin: 0 auto; max-width: class="mce_SELRES_start">

Menopause and hormonal changes

Menopause represents another important period of breast transformation.

The decline in oestrogen levels can alter both breast composition and volume.

In some women, breast involution may occur, with loss of volume and increased sagging. In others, the fatty component may increase, resulting in larger breasts.

These changes help explain why breast-related needs and preferences may change over the years.

Ageing and loss of elasticity

Over time, tissues can lose elasticity and their ability to provide support.

These changes can progressively alter the position and shape of the breasts and contribute to increased breast sagging.

The way the breasts evolve will differ from one woman to another and will also depend on skin quality and the other factors that have affected the breasts throughout life.

Weight changes and lifestyle

Weight fluctuations can also alter the volume and tension of breast tissues.

Increases and decreases in volume can cause changes to the skin and breast shape, particularly when they are significant or repeated.

For this reason, the assessment for breast surgery should take into account not only what the breasts are like at the time of consultation, but also the changes they have previously undergone.

What options are available to improve breast shape?

There is no single treatment for changing breast shape or volume.

The choice will depend on the characteristic that the patient wishes to address:

lack of volume

sagging

excessive size

tuberosity

asymmetry

or a combination of different factors.

Breast augmentation or augmentation mammoplasty

Breast augmentation can primarily correct a lack of volume and, in certain cases, also improve breast shape.

It may be used, for example, in patients with limited breast development, certain types of tuberous breasts, or mild sagging.

The technique should be selected according to the patient’s anatomy and the desired outcome.

Mastopexy or breast lift

Mastopexy can lift and reshape a breast that has descended.

It can be performed with or without implants. It can also form part of breast implant removal surgery, in which the implant is removed and the breast is reshaped.

In certain cases, fat transfer may also be performed, using fat obtained from another area of the body to add volume or improve specific areas.

Mastopexy generally involves more extensive scarring than breast augmentation, but it allows for greater reshaping when significant breast sagging is present.

Correction of tuberous breasts

The treatment of tuberous breasts is particularly variable because not all cases present the same characteristics.

In some cases, augmentation alone may be sufficient. In others, a breast lift, glandular reshaping, hybrid augmentation, or even different techniques for each breast may be required when asymmetry is present.

The choice should be made after individually assessing the shape, volume, and degree of tuberosity.

Breast reduction

Breast reduction can reduce breast volume and reshape the breast.

It usually involves a vertical or inverted-T scar, depending on the characteristics of the individual case.

Although the scars may be more extensive, breast reduction allows the surgeon to work with the patient’s own tissue to achieve a more proportionate breast volume and improve discomfort associated with excessively heavy breasts.

In addition to changing breast shape, many patients seek greater comfort in everyday activities through this surgery, such as getting dressed, exercising, or wearing certain types of clothing.

How do you choose the right treatment for your breast type?

The first step is to have an assessment with a plastic surgeon specialising in breast surgery.

It is not enough to identify whether the breasts are small, large, sagging, asymmetrical, or tuberous. To plan surgery correctly, it is also necessary to assess the structure of the chest, the width of the back, the position of the nipples, the inframammary fold, skin quality, and the amount of existing glandular tissue.

Communication between the patient and surgeon is also essential.

The patient should be able to explain what she would like to change and the result she hopes to achieve, while the specialist should clearly explain what can realistically be achieved based on her starting anatomy.

Not every result is possible for every body. Skin quality, the degree of sagging, existing volume, and body proportions all influence what can be achieved through surgery.

<picture></picture>

Is there an ideal breast type?

There are aesthetic proportions used as reference points in breast surgery, such as the approximate 45/55 distribution, with greater volume in the lower pole than in the upper pole.

However, the concept of an ideal breast should not depend solely on a mathematical proportion or on the aesthetic trends of a particular moment.

Very large breasts may be fashionable during a certain period, while years later preferences may shift towards more subtle and natural-looking results.

For this reason, when planning breast surgery, the aim should be to achieve the best possible result based on each patient’s individual anatomy.

Shape, size, and volume should be in proportion to the patient’s physical structure, chest, and overall body proportions.

Rather than pursuing a particular breast type, the aim is to achieve a harmonious, natural result that is consistent with each woman’s body, avoiding decisions based exclusively on temporary aesthetic trends.

breast enlargement, <a
